Project Description


Work in accordance with established policies and procedures and under the direction of the Health Center Administrator.  This person is responsible for providing social services/admissions/discharge arrangements in the Assisted Living and Health Center assisting residents and families in care services, insurance and to find resolution as outlined below function.

  • Complete social services documentation in resident files or medical records according to facility policy, state and federal regulations including care plans.

  • Participate in Health Center resident care conferences. Invite residents and family members to attend. Notify staff members involved.

  • Attend monthly Care Associate and In-Service meetings.

  • Contributes to a positive, healthy and safe work environment.

  • Assess residents for potential psychosocial needs to develop a plan with resident to arrange for internal and/or external professional assistance.

  • Identify residents who may need increased supportive care. Assist residents and families through transitions. Communicate with DON and Administration to determine best level of care for resident and how transition may be accomplished. Communicate to all disciplines the details of the transition and coordinate details.

  • Coordinates admissions to the Health Center. Follow up with residents after the admission to assess adjustment, answer questions, and assist with problem resolution. Complete admission face sheet (Vision) for MDS admission. Meet with resident to secure advanced directives, and/or aid residents to education desires of Advanced Directives.

  • Act as resident advocate; to protect resident rights as set forth by the Resident Bill of Rights. Accept resident and family grievances and facilitate grievance procedure.

  • Provide discharge planning and follow-up with residents after discharge from the facility. Complete discharge tracking record for MDS for all Health Center discharges or deaths.


Required Qualifications

  • Minimum Bachelor’s degree in Social Work or related field.
  • Currently licensed Social Worker in the State of Iowa.
  • Ability to work with sensitive/confidential material.
  • Must be able to pass pre-employment physical, drug screen and criminal background screen.

Preferred Qualifications

  • Experience with elderly and long-term care.
  • Excellent time management skills with a proven ability to meet deadlines.

Physical Requirements/Working Conditions:

  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift fifteen (15) pounds at times.
  • Physical effort is minimal.
  • Seventy-five percent (75%) of the work day is spent sitting, standing intermittently and bending.
  • Must be able to lift and carry up to fifty-one (51) pounds, periodically.
  • Requires manual coordination to operate computer and other business equipment such as fax machine, phone and photocopier.
  • Requires continuous mental and visual attention to diversified operations.
  • Must be able to function efficiently despite distractions and interruptions.
  • Communicates orally and/or in writing with residents, coworkers, families and public.
